Why make things complicated? Put a mattress on the ground, take 5 pieces of fabric and one big black piece of black fabric for the background. That was all that was used for the setting of this photo. The pose of the model makes a nice line in the photo that keeps the attention of the viewer on her face.
Offcourse the light helps with that. A flash on the left with a big relfector and an added grid kept the light pretty concentrated. It keeps her legs a bit in the dark. But let's be honnesed; the focus shouldn't be on her legs.
